Office Information
9:00 a.m. - 4:00 p.m. Monday through Friday Note: Dr. Faires sees patients all day on Mondays and in the afternoon on Tuesdays. Emergency situations which require the doctor's attention must take priority over regular appointment slots, and we will notify you if this is the case.Billing & Insurance
In order to ensure smooth handling of claims, it is essential to provide us with complete and accurate insurance information. Please bring your insurance information with you each time you come to our office.
Plastic surgery procedures that are typically covered by insurance include breast reconstruction, carpal tunnel syndrome procedures and Moh's closure. Please check with your insurance company to see if it will cover your procedure. If your surgery is covered, ask your primary care physician for a plastic surgeon referral. If your insurance does not cover your procedure, we request that you please pay in advance. We accept most types of payments.
Medical records, disability forms and narrative reports are available at Faires Plastic Surgery. A copying or dictation fee will be charged, based on the amount of material reviewed or copied.
